Enrollment Process for New Students

Welcome to Carbondale High School! 

To enroll a new student in the CCHS 165 District you will need to complete the following steps:

RESIDENCY

Legal guardians are required to provide documents for both student enrollment and residency.  Before enrolling a student, the legal guardian must provide proof of residency.  Illinois School Codes states that a child is entitled to enrollment in a district wherein he/she resides.  The code further defines residence as physical presence with an intention to remain on a permanent basis with the person(s) who has legal custody of the student. 

Proof of residency in the CCHS 165 District requires three pieces documentation-matching addresses of residence.  Click here for items that meet the proof of residency criteria.

In addition to the proof of residency documents, you must also complete the Parent/Guardian Authorization & Affidavit or Residence form.

If you and your child are living full-time with another family member, please bring that person with you (if possible) along with the Statement By Owner of Residence form and/or a letter from the person you are residing with. This form must be signed by a notary.

If your child is living full-time with another family member, the Evidence of Non-Parent Custody form must be completed and notarized.  See the attached link for the form.

ADDITIONAL DOCUMENTS FOR REGISTRATION

You will also need to provide the following documents for enrollment:

Certified Birth Certificate

Legal Custody or Court Order if applicable

Special Education/Special Needs documents – IEP or 504

Health Records – All incoming freshman and new-to-the district students must have current school physical on file with our school nurse.  See link below for the approved school physical form if you do not currently have an up to date physical.

Student must also have up to date immunizations that meet Illinois State Requirements.  See attached link for further information.

All CCHS students pay a $50.00 school fee unless you complete and are approved for the National School Lunch program.  School fees are for textbook rental, identification card, a first year PE uniform, locker rental, lab and material fees.  Some courses may require an additional fee through the school year, but the majority are covered in this cost.   

If you complete and are approved for free or reduced lunch, your school fees are waived.  Click the link to access the free and reduced lunch application.

If you do not plan to apply for the free/reduced lunch program, please plan to pay your student’s school fee of $50.00
